Membership Program

Recommended for children from 3 years old up to 6 years old: Includes one annual physical, with developmental evaluation, lead/HB, PPD, vaccines, vision and hearing evaluation school form completion; 2 sick visits in person/telehealth; and 2 in-house tests. Influenza vaccine is included in the plan as a nurse visit.  Price $420/year. $35 per month for your first child and $30 per month after the first child on this age range.  After 2 sick visits, there will be a charge between $75-100 for each new sick visit. Outside Laboratories extra fee may apply.  

Recommended for Children from 7 to teens and young-adults Includes one annual physical, mental health evaluation, PPD, vaccines, school form completion, sport physical, vision and hearing evaluation, outside laboratories for annual physical, plus a follow-up visit to review labs; 2 sick visits in person/telehealth; and 4 in-house tests (streptococcus, influenza, urinary analysis, pregnancy test, etc.) Influenza vaccine is included in the plan, as nurse visit.  Price $720 /year. $60 per month, for first child, and $50 per month after the first child on this age range.  Each new extra sick visit will be charged between $75-100. Teen-young adult lab package: $150 (includes STI testing, drug-testing urine and pregnancy test).  

For children younger than 3 years old and older children/teens or young adults with chronic medical needs, such as for asthma, ADHD, mental health disorders, special needs. Includes all well child visits per year and all services required described above appropriate for age, plus sick visits as needed for the child. Up to a maximum of 8 total visits in a year in person or by telehealth. Also, included in visits are parent consultations for counseling with the pediatrician and in-house and outside laboratories for annual physical exam and follow-up labs (total 2 of outside lab/year). Price $1200/year. $100 per month for first child, and $85 per month after the first child on this age range.  Special labs not included, but we negotiate with outside laboratory companies for special rates.  We offer another service for children and their families affected by weight problems. Our weight management program is open to all with and without insurances, and open to the community in general. Our physicians are board-certified in obesity medicine and our health-coach has over 20 years of experience. This is a cash-only program, health insurances do not pay for weight management services.
