We are excited to announce that we have earned a National recognition in Patient-Centered -Medical Home (PCMH) with a Distinction in Behavioral Health Integration (BHI) given by The National Committee for Quality Assurance (NCQA). Those recognitions are highly regarded at national level as a warranty of good quality service for patients in their medical home. The NCQA Recognition highlights our practice as one that is medically evidence-based in order to deliver our medical care and work to improve our quality of service. PCMH is a model where patient-centered protocols are created and implemented, facilitating the teamwork on the discussion and decision-making processes for each patient. PCMH also integrates information technology to improve our quality of care , customer service, medical decision-making, continuity of care, more personalized care, health education and lower healthcare costs. As a PCMH center, we help to coordinate patient care with other health care systems, such as hospitals and urgent cares.
